Preparing for Your DOT Physical Examination

Complete the Health History Questionnaire

Fill out all sections completely and accurately. Include details about past surgeries, chronic conditions, medications, and any symptoms you're currently experiencing. Don't leave any required fields blank.

Review and Sign the Consent Form

Read the consent form carefully, ensuring you understand the authorization you're providing. Sign and date the form in the designated areas.

Bring a valid, government-issued photo ID (driver's license, state ID, or passport). The examiner will verify your identity before conducting the examination.
